Three crew members died when the helicopter crashed on April 26 in Hazelhurst. The NTSB released its preliminary report this week. The report does not determine what caused the crash.
The report says the Ascension Health Spirit helicopter had undergone 100- and 600-hour safety inspections a day earlier. The pilot, 34-year-old Rico Caruso of Hazelhurst, had more than 3,200 hours of flight time and had completed additional training earlier this year.
The crash killed Caruso, 30-year-old flight nurse Klint Mitchell of Watersmeet, Michigan, and 43-year-old paramedic Greg Rosenthal of Mosinee.
No patients were on board. The investigation is ongoing.
