Wausau Pilot & Review

The following are local health inspections conducted from January 1, 2024, to January 31, 2024. The establishments range from cafes and pizzerias to bars and grills, each with its inspection date listed, ensuring compliance with health standards. All information is courtesy of the Marathon County Health Department.

Becca’s Cafe, 4002 Schofield Av, Weston, 54476, inspected on January 26, 2024. No violations found.

Best Western Plus, 803 W Industrial Park Av, Rothschild, 54474, inspected on January 23, 2024. Another location at 201 N 17th Av, Wausau, 54401, inspected on January 31, 2024. No violations found.

Blue Willow Cafe, 1111 N 4th Av, Wausau, 54401, inspected on January 8, 2024. No violations found.

Buffalo Wild Wings, 1819 W Stewart Ave, Wausau, 54401, inspected on January 31, 2024. Four violations:

  • Label spray bottle by cook line as to contents.
  • An irreversible registering thermometer is not provided for measuring utensil surface temperatures in the hot water mechanical warewashing operation.
  • The 3 door cooler in back of kitchen needs cleaning to remove spilled product.
  • The floor in the kitchen is in need of repairing. Cracked tiles, missing grout, water not draining.

Cafe 529, 529 N 3rd St, Wausau, 54403, inspected on January 9, 2024: Inspectors noted 12 violations.

  • Marinara sauce in the steam table was hot held at 87.3°F. Marinara was discarded during the inspection.
  • (CORRECTED DURING INSPECTION):  House Italian salad dressing in the Superior 2 Door Cooler has exceeded its date mark; 8/30/2023. Discarded during the inspection.
  • The sanitizer concentration in the warewahsing machine was observed under 50 ppm chlorine. Call for service.
  • (CORRECTED DURING INSPECTION):  The sanitizer concentration in the wiping cloth pails was observed at under 50ppm chlorine.
  • The Person in Charge (PIC) is not monitoring the food temperatures for foods during hot holding. This is regarding the observation of the marinara sauce being hot held on the steam table at 87.3 F.
  • The Person in Charge (PIC) is not monitoring proper sanitization techniques in the food establishment. This is regarding wiping cloth pails and the warewashing machine sanitizer concentrations being below the allowable limit of 50 ppm.
  • Several temperature control for safety (perishible) food in coolers were not date marked. An example was the raspberry italian salad dressing in the Superior 2 Door Cooler.
  • The walk-in cooler is not sufficient in capacity to hold foods at temperatures below 41F. Call for service.
  •  Olive spread, in the Superior 2 Door Cooler, was not labeled with the name of the food.
  • (CORRECTED DURING INSPECTION):  The Sandwich Prep Table was not equipped with an integral or permanently affixed thermometer.
  • The shelving, under the counter across from the Sandwich Prep Table, needs to be cleaned.
  • (REPEAT):The vinyl tile flooring, in front of the Sandwich Prep Table, is cracked and pieces are missing.

Cantina Boys Tacos & BBQ, 201 N 17th Av, Wausau, 54401, inspected on January 31, 2024. Three violations:

  • Rice and Coleslaw etc in cold holding units are not date marked.
  • A quat test kit is not available for checking sanitizer concentrations.
  • Food containers are not labeled with common names.

Chellis St Pub, located at 265 Chellis St, Wausau, WI 54401, was inspected on January 31, 2024. No violations.

Day Break Bar & Grill, located at 5307 Westfair Av, Weston, WI 54476, was inspected on January 23, 2024. Five violations:

  • Must properly identify all items on your menu that may be served undercooked. (burgers , eggs, etc.)
  • The drop chute inside the ice machine needs cleaning
  • Box of ribs stored on floor in walkin cooler, box of potatoes stored on floor in storage room.
  • When cooling items like soups, biscuit gravy remove from cooking container and place in a metal shallow pan. This will cool product faster.
  • An irreversible registering thermometer is not provided for measuring utensil surface temperatures in the hot water mechanical warewashing operation.

Domino’s Pizza, located at 301 S 2nd Av, Wausau, WI 54401, was inspected on January 17, 2024. Three violations:

  • Pole used for eliminating air bubbles in crust and food thermometers not cleaned at a frequency to preclude accumulation of soil residue.
  • On the receiving door there are drilled holes of approximately a quarter inch diameter near the door handle and a gap of approximately 2 inches on the lower left side letting in daylight.
  • The floor area under the counter cooler and prep tables contains a significant accumulation of dirt, debris, and food pieces creating insanitary conditions. Areas of the walls harbor food debris and residue, such as opposite the warewashing sink.

Edgar Family Restaurant, located at 127590 Opportunity Ln, Edgar, WI 54426, was inspected on January 24, 2024. 16 violations:

  • Shell eggs were found stored above uncovered non-raw items such as chili, and cooked pasta in water in kitchen reach-in cooler.
  • Kitchen workers were observed washing their single-use gloves in the handwash sink rather than discarding them and washing their hands between uses.
  • (CORRECTED DURING INSPECTION):Cream of broccoli soup in large pot was found left on stove with no heat, and was at a temperature of approximately 85 F at time of inspection.
  • Bottles of tartar sauce in condiment cooler were found to be cold held at approximately 50°F.
  • Bottles of tartar sauce were found be held at approximately 50 F. Person in charge stated that the cooler had not been cooling properly and that the bottles of tartar sauce were going to be disposed of. There was no indication for other employees on the bottles or the cooler that these unsafe items were not to be used, or that the unit was not functioning properly.
  • Food prep sink in rear segment of kitchen hard-plumbed and thus lacks air-gap necessary to prevent contamination in the event of backflow.
  • Facility management failed to provide food safety training to designated PIC sufficient to maintain regulatory compliance in their absence relating to the following areas: sanitization methods, monitoring of temperature-sensitive foods for hot-holding and cold-holding, proper date labeling, and the prevention of adulteration during storage.
  • Cases of frozen chicken meat and ground beef patties were found to be thawing on the kitchen counter. Employee stated that this is the usual method for thawing such items at the facility.
  • A variety of food items across the facility including raw pork loin, raw chicken wings, cooked pasta, portioned condiments, potato salad, and other items in the walk-in cooler, reach-in coolers, and counter coolers were either lacking date labeling or had incorrect/long-expired dates.
  • Various food items in the freezer, walk-in cooler, and reach-in cooler were observed to be stored unwrapped or uncovered, including a large bowl of potato salad, multiple containers of cooked pasta, an open case of vegetable mix, and others.
  • Blue buckets used for food storage are not made for long-term use as they are not made of a sufficiently durable material, and are not a cleanable surface. All direct food-contact containers must be NSF/ANSI certified.
  • Cutting board on north side of cookline covered with stained pits and scratches that cannot be removed by washing. Cutting board can no longer be sufficiently cleaned to preclude the harborage of food matter and microbes.
  • Shelving and bottom of ice cream freezer where glasses are stored lip-down contain dust and debris.
  • Drinking glasses are stored lip-down on visibly dirty shelving and on the bottom of the ice cream cooler. Additionally, small bowls being used as scoops in the crouton container and a container of seasoning are inqdequate to prevent cross-contamination as the handled portion of the surface directly contacts the food.
  • Fluorescent lights in front of house prep area not shielded, coated, or otherwise shatter-resistant.
  • Kitchen back door has gap on lower right side that fails to prevent pests from entering facility.

El Mezcal Mexican Restaurant, located at 3100 Rib Mountain Dr, Wausau, WI 54401, was inspected on January 24, 2024. Inspectors noted seven violations:

  • (REPEAT) (CORRECTED DURING INSPECTION):  Eggs and raw meats were observed being stored over ready to eat salsa and chili verde in the walk-in cooler.
  • (REPEAT) (CORRECTED DURING INSPECTION):  Washed produce was observed being stored under unwashed produce.
  • Chili verde in prep table cooler was observed being cold held at 44.1°F.
  • The Person in Charge (PIC) has not provided food safety training to employees on the proper organization of coolers so that Ready to Eat (RTE) foods are being stored above raw foods such as meats and unwashed produce.
  • Prep table cooler is not sufficient in capacity to hold foods at cold holding temperatures below 41F.
  • No test kit is available for monitoring the Sink & Surface Sanitizer concentration.
  • Thermometer in the prep table cooler was broken and not easily readable.

Evelia’s Restaurant, located at 3406 Schofield Ave, Schofield, WI 54476, was inspected on January 30, 2024. Three violations:

  • Must date mark all ready to eat foods held in the coolers if not used within 24 hours.
  • Some items in the menu shall not be identified by the consumer advisory, chicken dishes, vegetarian melt,turkey melt. Chnage next time a new menu is printed.
  • Store bag of onions off floor by 6 inches and not underneath the handwash sink.

Glass Nickel Pizza Company at 227255 Rib Mountain Dr, Wausau, WI 54401, inspected on Jan. 9, 2024. One violation:

  • (REPEAT):Employee observed working in pizza prep area without a hair restraint.

Olive Garden Italian Restaurant (#1864) at 226620 Rib Mountain Dr, Wausau, WI 54401, inspected on Jan. 10, 2024. 3 violations:

  •  Ice bins are not properly air gapped.
  • The faucet end of the mop sink is not provided with a backflow preventer.
  • Lights located in Produce Walk-in cooler are not shatter resistant or shielded.

One More Cast Grill & Bar at 140 N Front St, Unity, WI 54488, inspected on Jan. 17, 2024. Eight violations:

  • (REPEAT) (CORRECTED DURING INSPECTION):  Condiment cups with date marks of 12/29 were still present in the refrigerator.
  • Food items such as what ppeared to be turkey and gravy in the kitchen refrigerator was not labeled or date marked.
  • No handwash sink is present in kitchen for use during food preparation.
  • There is no approved or current food protection manager’s certificate posted in the food establishment.
  • Observed taco bell food in kitchen refrigerator and canned goods that are for personal use only in walkin cooler.
  • (REPEAT):Handwashing sink is not non-hand operated in bar area.
  • (REPEAT):There is no service sink provided in food establishment.
  • Floor and wall juncture along the side and back walls of the kitchen are not coved.

Reliance Inn at 116 S 17th Av, Wausau, WI 54401, inspected on Jan. 31, 2024. Two violations:

  • No test kit is available for monitoring sanitizer concentration.
  • Refrigerator is not equipped with an integral or permanently affixed temperature measuring device.

Stratford Tiger Stop Cafe at 212804 Hwy 97, Stratford, WI 54484, inspected on Jan. 24, 2024. Eight violations:

  • (CORRECTED DURING INSPECTION):  Steaks are stored over ready to eat foods in the one door cooler and raw hamburgers are stored next tolettuce in the prep cooler.
  • The Person in Charge (PIC) needs to train employees in food handling practices (cooling, storage)
  • Salmon is being improperly thawed in the walk in cooler, must remove from packaging.
  • Consumer advisory is missing the disclosure statement. Not all items are marked correctly if serving them undercooked
  • A chlorine test kit is not available for checking sanitizer concentrations.
  • Flour is stored in unlabeled container in the back storage area.
  • Soups are being cooled in plastic containers, recommend transferring to shallow metal pans.
  • Provide self closing doors for the restrooms.

Subway #108 at 2409 N Mountain Rd, Wausau, WI 54401, inspected on Jan. 22, 2024. Five violations:

  • The Person in Charge (PIC) is unable to demonstrate knowledge regarding the importance of proper equipment cleaning procedures, to include washing with soap, rinsing with clean water, and then sanitizing.
  • Food establishment does not have procedures that address responding to vomiting or diarrheal events.
  • The handwashing sink was unavailable for proper handwashing, as containers holding sliced deli meats were set on both sides of the handwash sink.
  • (REPEAT):The food protection manager’s certificate is not posted in the food establishment.
  • Mops were observed being stored wet in the mop bucket.

Subway #119 at 212400 State Highway 97, Stratford, WI 54484, inspected on Jan. 24, 2024. No violations.

Subway #134 at 670 Maratech Av, Marathon, WI 54448, inspected on Jan. 18, 2024. Five violations:

  • (REPEAT):  The drain of the handwash sink, located to the left of the 4 compartment sink, drains very slowly and needs to be cleared.
  • (REPEAT):  The faucet at the 4 compartment sink is leaking and is in poor condition.
  • (REPEAT):  Vinyl tile in front of the receiving door is damaged and pieces are missing. This portion of the floor is no longer easily cleanable.
  • (REPEAT):The receiving door is not tight-fitting. Daylight can be seen underneath. Repair weather stripping.
  • (REPEAT):  Retail food establishment permit is not posted in a conspicous area in the food establishment.

Thunderbird Bar & Grill at 1217 Western Av, Mosinee, WI 54455, inspected on Jan. 31, 2024. Two violations:

  • Wiping cloth used for wiping surfaces stored on sink and not in a sanitizing solution.
  • (CORRECTED DURING INSPECTION):  The microwave and pizza ovens are not cleaned frequently enough.

Tine & Cellar at 3806 Schofield Av, Weston, WI 54476, inspected on Jan. 30, 2024. Three violations:

  • A chlorine test kit is not available for checking sanitizer concentrations.
  • Label all squeeze bottles as to contents located on the prep lines.
  • Replace or refinish the planks used to serve food as some are cracked or very rough surfaces.
